In making my contribution to the Boone for U. S. Senate campaign, I am affirming the following statements as true and accurate:
  1. I am not a foreign national who lacks permanent residence in the United States.
  2. I am not a Federal government contractor
  3. This contribution is made from my own funds, and not those of another
  4. This contribution is not made from the funds of a corporation or labor organization
  5. This contribution is made on a personal credit card or debit card for which I have the legal obligation to pay, and is not made either on a corporate or business entity card or on the card of another person
  6. I am at least eighteen years old
 
An individual can contribute as much as $2,500 to the Boone for U. S. Senate Campaign for the primary election and as much as $2,500 to the general election, for a total of $5,000 per person.
Contributions or gifts to the Boone for U. S. Senate campaign are not tax deductible.
Federal law requires us to use our best efforts to collect and report the name, address, occupation and name of employer of individuals whose contributions exceed $200 per election cycle.
Contributions to the Boone for U. S. Senate Campaign are subject to the prohibitions and limitations of the Federal Election Campaign Act. That law prohibits federal contributions from corporations, labor organization treasury funds, federal government contractors and foreign nationals who are not admitted for permanent residence. All contributions must be made from personal funds and may not be reimbursed by any other person.
